How do I collect service ratings?

Do customers need an account?

Should I ask more than one question?
Not at the point of sale. One question gets a usable response rate; three gets almost none. Add a conditional follow-up for low scores only.
Where should I put the QR code?
On the receipt, the counter, or a table card. Somewhere it's seen at the moment the service is fresh.
How is this different from NPS?
NPS asks about likelihood to recommend overall. This asks about a specific interaction. Use this for operational feedback, NPS for tracking loyalty over time.

How do I get customers to actually scan the code?

Where should I not put the QR code?
